Fourth Avenue Homes, Inc. is currently seeking a kind, compassionate and responsibleProgram Coordinator (PC) to supervise all aspects of the daily operation of a licensed group home in Willmar, MN. This location serves one adult male with physical disabilities who will need full assistance with all of his daily ADL'S. The ideal candidate would be an LPN or CNA although not required.

A PC would ideally have the ability to effectively supervise direct care staff to ensure the health, safety, and protection of rights of each person served in the program. The PC will also be responsible for overseeing the implementation of each person's served Coordinated Service and Support Plan (plan of care). ThePC will ensure that they and their staff are providing the necessary support to each individual to ensure that their needs, interests, and preferences are respected and provided for in a safe and healthy environment that maximizes personal success. ThePC must have the ability to communicate effectively, orally and in writing, and be able to create documents that are legible and accurate. A valid driver's license is required as well as the ability to pass a background study.

Job Type: Full time including sleep overnights. Pay is based per location. Perfect opportunity for college students and people looking for supplemental income.

Pay: $20.00-$22.00/hour per hour based on experience. Singing bonuses and referral bonuses available as well.
